It is our policy at Quinn Orthopedic Physical Therapy to confirm the identity of each of our patients via a photo ID check at the time of intake. Please bring a copy of your photo ID and insurance card to your first visit. We will take a copy of both insurance and identification in order to ensure that any charges from our office are billed properly to the appropriate entity and under eligible coverage. Thank you in advance for your understanding and cooperation with this policy. If you have a referral for physical therapy please also bring a copy at your first visit or have it faxed to our office at: 408-252-1159.
For your convenience we have provided links to the new patient paperwork required at your first visit. By printing and completing the documents linked here and bringing them with you to your first appointment you can save yourself some time in our waiting room.
