Insurance

We are a Participating Provider for Medicare, and a Preferred In-Network provider on the most common PPO plans in our area such as:

  • Blue Shield
  • Cigna
  • First Health/CCN
  • Great West
  • Guardian
  • Healthnet
  • Medicare
  • PHCS (Private Healthcare Systems)
  • Principal
  • Tricare/Triwest
  • UAS (United Aministrative Services)
  • United Healthcare
  • And Many Others

We are not currenly an in-network provider for Aetna or Pacificare, but we would be happy to bill them on your behalf as an out-of-network provider. Please call our office to find out what your out-of-pocket cost would be to use us as an out-of-network provider on one of these plans.

We do NOT accept Anthem Blue Cross due to what we consider to be unreasonably low reimbursement rates and unfair contracting policies. We welcome Anthem Blue Cross patients at our cash-pay discount to our standard fee schedule, and we will provide you with a standard "Superbill" form that you can submit to Anthem Ble Cross in order to try to receive partial reimbursement. Payment will be due at time of service.

We do NOT accept Medi-Cal or HMO plans, including Medicare HMO plans.

Auto Injuries

We'll be happy to bill your Auto Insurance plan for MedPay benefits in the event that your injury is the result of a motor vehicle accident. Please be aware that we do not accept legal liens in auto cases under any circumstances - in the absence of auto insurance medpay, we will be happy to bill your medical insurance on your behalf. Otherwise, direct payment will be due at the time of service.

Workers' Compensation

We'll be happy to bill your employer's Worker's Compensation carrier if your injury is work related.  Please be aware that all Workers' Compensation visits must be pre-authorized by the insurance carrier, and that this process can take a few days and is beyond our control.  We will do our best to get your authorization processsed as quickly as possible so that you can begin or continue your care without interruption.

There are many other insurance companies that are not listed above that we accept.  If you are not sure whether your insurance company wil pay for your physical therapy visits, please contact our office so we can help you determine your insurance coverage.

At the start of your care we will contact your insurance comnpany to determine the coverage guidelines of your plan.  We will explain those benefits to you and advise you of any expected liability on your part.  We will expect any deductible and/or co-payment amounts to be paid at the time of service.  We will also communicate with you throughout the course of your treatment to keep you advised on the status of your insurance company's processing of your claims.  We will be happy to bill your insurance company on your behalf, but please keep in mind that ultimate responsibility for payment of your bill is yours.

We accept cash, checks, VISA, Mastercard, American Express and Discover for payment of co-payments, deductibles, and patient balances.
